The platform will connect you with all of Roseville’s Wellness program tracks, the Benefits Contribution Incentive process and Gym Membership Attendance Incentive.

Roseville Wellness Program
Employees must verify their Benovate Connect platform in order to participate in the Benefits Plan Contribution Incentive, Roseville Wellness Tracks or Gym Membership Attendance Incentive. All forms will be submitted via Benovate Connect. A card will appear in the platform when submissions are being accepted. See below for deadlines and details.
Benovate Connect Platform
Benovate will be your one stop shop for accessing and submitting all Roseville Wellness Program materials. Employees on
the city’s health plan can earn up to a total of $200 in the platform - $15 for activation & completing the walkthrough, $35 for
Health Assessment completion, plus $150 for various platform activities. If you don't do the Health Assessment or have
already verified your account, you can earn those amounts through other platform activities. Employees must verify their
account in order to participate in other City of Roseville Wellness program elements. Employees that are not on the City of
Roseville health plan may still participate in the wellness program through the Benovate platform and get the incentives they
earn through the city’s program, but are not eligible for the platform incentive.
Benefits Plan Contribution
To earn $40 a month toward your full city benefits contribution you must complete a physical exam/screening, blood
pressure screening and the platform’s health assessment and submit proper documentation via the Benovate Platform by
noon on December 8, 2017 to be eligible to receive the full city contribution in 2018.
When combined with Biometric Screening data, Benovate’s online Health Assessment allows you to dive a little deeper into
your current health status and establish a baseline for maintaining and improving! You will be prompted to complete your
Health Assessment within the Benovate Platform, or you can take it any time by clicking "Personal Health Desktop" in the
menu, but the incentive for completing this is allowed once per year. This is also one of the requirements to receive the
Benefits Plan Contribution Incentive.
Roseville Tracks Program
Employees can earn 4 hours of their choice of PTO/Vacation, deferred compensation or taxable wages for completing one
track per trimester. Employees can choose from the General Wellness Track, Weight Loss Track or Tobacco Cessation
Track. Forms must not contain any health sensitive data and be submitted via the Benovate platform within the first 14
calendar days after the trimester ends, with the deadline for submital on the 14th at 4:30pm.
Gym Membership Reimbursement
Attend the gym of your choice at least 8 times per month to be eligible for $50 per approved monthly attendance . Health
club attendance record must be submitted via upload to the Benovate platform within the first 14 calendar days after the
trimester ends, with the deadline for submital on the 14th at 4:30pm.
Biometric Screening & Medical Review
The Biometric Screening only takes about 10 minutes and will be done onsite by a skilled phlebotomist. By securely
gathering health data like your height, weight, cholesterol, blood sugar, etc., you are able to see an accurate snapshot of
your health and potential health risks. Your results will automatically be loaded into your Benovate account within
approximately 10 days.
Once you have completed your Biometric Screening and results are loaded, you'll be prompted to schedule your telephonic
Medical Review. During your confidential call with the medical practitioner, you will have the opportunity to discuss care plan
options as well as recommended healthy activities based on your results. You must do both the screening and medical
review to receive the incentive.
The onsite screening & review take the place of the blood work portion of the annual physical only and will count as the
annual physical portion for the benefits incentive contribution. You can still see your physician for other annual physical
items such as mammogram or prostate checks as part of a wellness checkup. You may alternatively chose to do your entire
annual physical and submit the affidavit via Benovate to qualify for the benefits plan incentive contribution as you have in
years past if you wish. Note: Insurance will only cover either your annual physical OR the onsite screening & review as
your yearly preventative exam, not both and you must be on the city's health insurance plan to utilize this screening.
